I think Joff is just a horrible little monster, and its entirely plausible he just hated Ned for trying to keep him from ruling or just simply wanted to kill him because Joff likes killing/torturing people who challenge him in some way. He does it other times as well.
I don't think Cersei had anything to do with it, I always believe her surprise is genuine. Littlefinger might have had something to do with it, but I usually blame Joff in full.
